Disease Management Program

Diabetes and cardiac conditions are among the most common and highest-cost conditions that are covered by health benefits of BHCG member companies. BHCG instituted a Disease Management Program for these two conditions with American Healthways in 2003 after analysis of member company health data showed that 44 percent of total claims fell into these actionable disease categories.

Through BHCG’s Disease Management Program, participating companies receive collective pricing and aggregate performance guarantees. The program supports participating employees and dependents with a Patient-Centric Support Plan that includes introductory letter and welcome packet, general health assessment, screenings, regular care calls and reminder cards, and educational materials and programs.

The program was piloted by two large member companies in 2004 and several more members implemented the American Healthways program in 2005.
